What We Do

  • Texas Healthy at Home connects high-complexity clients to the services they need to stay safely at home. Our care coordinators conduct comprehensive assessments to identify social needs — including food insecurity, housing instability, lack of healthcare access, transportation barriers, and financial hardship — and then connect clients to vetted community resources that address those needs. In FY2025 we served 503 Texans through full care navigation, identifying an average of 5 distinct social needs per person.

  • Falls are the leading cause of injury among older adults — and in DFW alone, 28.1% of adults 65+ report falling each year. Texas Healthy at Home and WellMed Charitable Foundation co-lead the Texas Takes on Falls Coalition, a 60+ organization statewide network working to change that through evidence-based programming, workforce training, public awareness, and systems change. In 2024 THAH and coalition partners engaged 2,800+ DFW residents through Falls Free CheckUp assessments and connected them to local prevention programs and resources.

  • Caring for a person with dementia is one of the most demanding — and isolating — experiences a family can face. Texas Healthy at Home provides free educational webinars in partnership with James L. West Center for Dementia Care and Alzheimer’s Association North Central Texas Chapter for caregivers and professionals on topics including caregiver guilt, anticipatory grief, managing challenging behaviors, late-stage dementia care, and community resource navigation. Since 2021 our dementia education programs have reached 3,000+ participants across 385+ Texas zip codes — with 92% reporting increased knowledge after attending.

  • Texas Healthy at Home and Guardianship Services Inc. co-lead the Texas Elder Justice Coalition, a 150+ member statewide network dedicated to preventing elder abuse, neglect, and financial exploitation. The coalition brings together Adult Protective Services, law enforcement, financial institutions, legal advocates, and community organizations to detect, prevent, and respond to elder harm across Texas. Through education, policy advocacy, and coordinated systems response, we work to ensure that every older Texan is protected, respected, and heard.

  • Texas Healthy at Home and CATCH Greater Houston are partnering to advance the development and sustainability of the Community Care Hub model across Texas. The CCH model provides shared infrastructure that allows Area Agencies on Aging and community partners to modernize coordination, engagement, and outcome visibility — while preserving existing programs, systems, and regulatory requirements. This work is being approached conservatively and incrementally, with careful attention to risk management, compliance, and organizational readiness. Together THAH and CATCH are building the connective tissue that allows community-based aging services to evolve alongside shifting funding landscapes and healthcare expectations.
