GUIDE (Guiding an Improved Dementia Experience) Model

Free Help with Dementia Care

Living with dementia can be challenging, whether you are the person with dementia or a caregiver. But free help is available through a federally-supported initiative called GUIDE (Guiding an Improved Dementia Experience).  

GUIDE offers: 

  • Substitute care that gives caregivers a short-term break from caregiving duties and enables them to take care of their own personal tasks and errands. 

  • Practical training on the best ways to care for someone with dementia. 

  • Personalized care plans that adapt as needs evolve. 

  • Comprehensive assessments to understand the specific challenges and strengths of each individual and caregiver. 

  • Ongoing support with regular check-ins to help navigate the complexities of dementia care. 

Clients may or may not have a caregiver. 

Texas Healthy at Home is a GUIDE provider site that subcontracts with community-based organizations to provide these services.  

Eligibility Requirements:

  • Have formal dementia diagnosis 

  • Enrolled in traditional Medicare 
    (not Medicare Advantage) 

  • Not living in a long-term nursing home 

  • Not enrolled in a SCO, One Care, PACE, or hospice
